Please fill out the form below when a COMPLAINT is received from an employee or student at a school campus.


  1. The Vandelay employee who receives the initial issue should file the complaint below.
  2. The appropriate management member will handle the complaint.
  3. The management member will document the resolution of the complaint in Smartsheets, and will then notify the Vandelay employee of the outcome/resolution.
  4. The Vandelay employee then contacts the original person who filed the complaint with the final outcome/resolution.
